About Your Benefits
By knowing your numbers, you have the power to determine the most important areas to focus on to reach your health and wellness goals.
Biometric Screenings are not required but are strongly encouraged.
Members can earn 5 HealthQuest Credits for completing an annual Biometric Screening.
Members can also earn an additional 6 Credits for measuring within the healthy incentive range for:
- Blood Glucose
- Blood Pressure
- Cholesterol
Ways to Complete a Biometric Screening
SEHP members may complete an annual Biometric Screening in any of the following ways:
- Attend an Onsite Screening Event
- Order and submit a Home Test Kit
- Schedule an appointment at the HealthQuest Health Center by Marathon in Topeka, KS
- Have your physician complete the Physician form at your annual well exam

Onsite Biometric Screening Events
HealthQuest sponsored screening events are offered in many locations statewide for convenience. Screenings provide you with important health information, including:
- Total cholesterol, HDL, LDL, and total cholesterol to HDL ratio
- Triglycerides and glucose
- Systolic and diastolic blood pressure
- Height, weight, and body mass index (BMI)
- Waist circumference

Home Test Kits
At-home screening kits are available to those who do not have access to a biometric screening event in their area. A test kit is mailed to your home upon request. You collect your own blood sample through a simple finger stick test and return your sample in a self-addressed, stamped envelope. Your results are automatically loaded within 15 business days of returning your sample.
PLEASE NOTE: The at-home test kit does not include results for blood pressure, height, weight, and waist circumference.
To order a Home Test Kit, visit the Kansas Wellness Portal. Choose the Home Kit Option. You are not able to order a Home Test Kit if you already have an appointment for an onsite screening event.

In-Person at the HealthQuest Health Center by Marathon
Covered Employees and Spouses are eligible to receive a Biometric Screening at the HealthQuest Health Center, operated by Marathon Health, located in Downtown Topeka, free of charge.
Biometric Screenings at the Health Center are eligible to receive 5 credits, in addition to 6 credits per Healthy Range Incentive you measure within, for a possible maximum of 32 credits!
You can register for a screening by calling the HealthQuest Health Center at 785-783-4080 Mon, Wed, Fri 7am-4pm; Tu, Th 9am-6pm.

Physician Form
You receive your HealthQuest Credits for an annual biometric screening by having your physician complete the Physician Form at your annual well visit. The State Employee Health Plan will cover one preventive exam per calendar year in full (with no co-pay). Just print the Physician Form and take it with you to your appointment. Employees may use test results from visits occurring between January 1, 2022, and December 30, 2022.

Reasonable Alternative Workshops
Did you complete a Home Test Kit, or measure outside of the healthy incentive range for glucose, blood pressure, or cholesterol? No worries, you can complete the CCE Controlling Blood Pressure module, CCE Managing Your Blood Sugar Module, and CCE Preventing High Cholesterol module in your HealthQuest portal for 6 credits each, to make up for testing outside of the healthy incentive range.
You can find these modules by clicking “Earn Your Incentive Credits” on the left side of your HealthQuest portal dashboard, and viewing activities under “Healthy Range Incentives + CCE Reasonable Alternatives.”
