1095C Tax Forms
January 2026 - The 1095C tax forms will be available in the MAP Member Portal by the end of the month.
Starting with the 2024 plan year, employers are no longer required to mail printed 1095C forms unless an employee specifically requests one.
Employees who need a printed copy have two options:

Download and print the form from the MAP Member Portal.

Request a copy from SEHP by submitting a communication form request in the MAP Member Portal.
Non State HR Professionals
Non State HR professionals with MAP HR Portal access can retrieve their employees’ 1095C forms from the HR Portal.
