Identification Card Information
New Employee
If you are a new enrollee or have made a coverage level change, medical, dental and voluntary prescription eyewear identification cards will be mailed to you.
If you are a new enrollee for voluntary benefits, an identification card will be mailed to you.
If you have a Health Savings Account, Health Reimbursement Account or Flexible Spending Accounts, the programs are managed by MetLife, and one identification card is used for all three accounts.
